Forms required depend on the type of divorce you are filing.  

You will need Form 8A (Application for Divorce) for a simple or joint divorce. Additional forms such as a financial statement (Form 13) or a parenting affidavit (Form 35.1) may be required if you’re dealing with child custody, spousal support, or property division. All forms can be downloaded online or collected from a courthouse.
