You will need key forms like Form 8A and Form 36.  

For an uncontested divorce, the main forms include Form 8A (Divorce Application), Form 36 (Affidavit for Divorce), and Form 25A (Divorce Order). You may also need a copy of your marriage certificate and possibly a Registration of Divorce Proceedings form to notify the Central Registry.
